This screen shot is from a MS Visual Studio C++ Express survey of SecureStudies.com. This may be the 5th or 6th time I'm seeing a "None of the above" radio button with checkboxes.
And I dont want to name but, a few of my encounters were from UI design pioneers like Apple and Google!

But the worst part with this survey was not this issue. after answering 53509385 questions one by one, in the final step there came a message: "There is something wrong occurred while processing. Please try again later.", Huh are you kidding? And there was no go backwards option, no lifesaver! I never felt that appreciated after spending mins on filling out the every question.
